20. WEBINAR - Building the evidence for Case Management - 16th October 2025

In the world of case management, securing funding for truly individualised rehabilitation programmes can be a significant challenge - especially when clients face pre-existing social or health inequalities, or, like 17.1% of the UK population (9.6 million people; gov.uk, 2019), live in rural areas where access to rehabilitation professionals is limited.

For case managers, presenting robust evidence to justify bespoke rehabilitation plans - particularly when they fall outside standard NHS therapy offerings - can be difficult. Recognising this gap, Bridge Case Management has developed a solution: an outcome measure tool specifically designed to capture the complexities and unique needs of individual clients.

CM-TOMs is a tailored outcome measure tool created by Emma and Siân, in collaboration with Professor Pam Enderby from the University of Sheffield. It is designed to demonstrate the measurable impact of case management on a client’s rehabilitation journey, building a solid evidence base to support truly personalised recommendations.

In this session, we will:

  • Introduce the CM-TOMs framework and its practical application

  • Share preliminary findings from Bridge’s use of the tool

  • Explore how this evidence can be used to advocate for client-centred, needs-led rehabilitation support

Meet the Presenters

Emma McClean, Clinical Director
Emma qualified as a nurse in 1998 and began her career in Neuro Critical Care within the NHS. After several years working in this high-acuity setting, she played a key role in the rollout of one of the UK’s first stroke thrombolysis services.

Following this, Emma was instrumental in establishing a slow-stream, community-based neurorehabilitation service, further deepening her expertise in neurological recovery and long-term care pathways. She later transitioned into case management, where she developed a strong reputation for working on both single and joint instruction cases, specialising in acquired brain injury (ABI) and complex multi-trauma.

Emma briefly stepped away from case management to lecture undergraduate nurses and nurse associates, earning a Postgraduate Certificate in Higher Education during this time.

She returned to case management with renewed focus and was appointed Clinical Director at Bridge Case Management in March 2025. Emma continues to combine clinical insight with a commitment to evidence-based, client-centred rehabilitation planning.

Siân Bailey, Clinical Lead
Siân qualified as an Occupational Therapist in 2004, beginning her career in the NHS with roles in learning disabilities and later as a Senior OT in elderly rehabilitation.

After moving abroad with her young family, Siân founded a private, school-based paediatric OT practice in Brunei in 2015, providing tailored therapy to children in educational settings.

In 2018, she transitioned into rehabilitation case management, supporting clients with catastrophic injuries. During this time, she developed expertise in coordinating complex care packages and achieved a Level 5 qualification in Health and Social Care Management.

Siân joined Bridge Case Management in 2021, where she expanded her clinical knowledge in acquired brain injury, orthopaedic trauma, and amputation case management. Her leadership and client-centred approach led to her promotion to Clinical Lead in June 2024.
