2016 CMSUK Case Manager of the Year Award

We are proud to launch the 2016 CMSUK Case Manager of the Year Award and to this end we need your nominations!                            

Please take a moment to read the important information below pertinent to all nominations.

Award Criteria:

  • ‘Good outcome from good practice’
    • All nominees must be current CMSUK Case Manager or Associate members
    • Submissions should be no more than 1200 words but can be supported by testimonials from other parties
    • Submissions must demonstrate evidence of application to at least 6 of the CMSUK Standards of Practice
    • The standards chosen should be identified on the accompanying form
    • Submissions in all categories should provide clear evidence of a stepped care or goal orientated approach, measurable outcomes and evidence based practice.

Guidelines:

  • Nominations can come from anyone involved in the case example Solicitors, Insurers, Employers and other Case Managers.
  • Current CMSUK Directors cannot be nominated for this award.
  • You cannot nominate yourself or a member of your family.
  • The Nominee submission must be no more than 1200 words but can be supported by testimonials from other parties.
  • Please do not provide any personal details of any individual other than the nominee.
  • The decision of the CMSUK Board is at its absolute discretion and is final.  There is no right of appeal against any decision.
  • Judging will be undertaken by an independent sub committee of the CMSUK Board. Judges will use their knowledge of case management, the CMSUK standards and their technical skill to evaluate the information provided within the application and supporting documentation. If the CMSUK Board feels that any Director has a conflict of interest that Director will not take part in the judging process.
  • The CMSUK Board reserve the right to select the category finalists as deemed necessary.
  • The finalists for the overall Case Manager of the Year Award 2016 will be invited to attend the 2016 CMSUK Annual Conference, AGM and Dinner Dance at the expense of CMSUK.
  • Presentations from each category will take place at the CMSUK Conference Dinner on Thursday 10th November 2016 and an overall winner will be announced on the night.
  • Closing date for nominations is Monday 12th September 2016.
  • All nominees will be notified whether they are through to the final by Friday 30th September 2016.

How to Nominate:

  • CLICK HERE to download the application form.  Alternatively, email info@cmsuk.org and request the documentation is emailed back to you by return.
  • Submit, in no more than 1200 words, attached to the completed form, your nomination demonstrating evidence of application to at least 6 of the CMSUK Standards of Practice. This can be supported in addition, by testimonials from other parties.
  • Nomination forms should be returned to: Heather Roberts, by email to heather.roberts@cmsuk.org, or post to CMSUK PO Box 293, Sutton, SM1 9BH.
  • Closing date for submissions is Monday 12th September 2016.
  • Please could all nominators and nominees ensure that the date of the awards is in their diaries.

  Prize Information:

  • The Award finalists will be short listed from the nominations and will be invited to attend the CMSUK Annual Conference & AGM 2016 'Technological Advances in Case Management’ as guests of CMSUK.  The venue for this showcase event is The Nottingham Belfry Hotel, Mellors Way, Nottingham, NG8 6PY on Thursday 10th  November 2016.  The package the finalists will be offered will include delegate attendance, accommodation at the Nottingham Belfry Hotel on 10th November and attendance at the CMSUK Dinner Dance also on 10th November where the ultimate winner will be presented with the 2016 CMSUK Case Manager of the Year Award.
